This community health improvement plan (CHIP) is the foundation of SHAPE Riverside County, a community-wide initiative that aligns public and private resources to improve health for all in Riverside County. It is a long-term, systematic plan to address issues identified in the Community Health Assessment (CHA). The purpose of the Community Health Improvement Plan is to describe how partners and the community will work together to improve the health of the population of Riverside County.
